EPR Terms, Acronyms, Frequencies and Abbreviations

Visit Types

Event Status

Many of the below terms are applicable to lab events only.

Cancelled Test cancelled
Collected Specimen collected
Completed Results entered
Corrected Original test results corrected
In Progress Specimen received by Lab
Partial Partial results entered by Lab
Resolved Procedure expired by the computer
Scheduled Test has been ordered
Supp/Corrected Original result supplemented or corrected
Unverified Results to be verified (e.g. Patholgy Reports)

 

Event status can be viewed in various locations in EPR, including Chart Review and Patient Care Schedule.

Order Status

The below terms may apply to lab, medication, diet and other orders.

Active Order sent, results not yet available (for lab orders)
DC'd Order has been discontinued
DC'd with events Order discontinued with outstanding events
Expired All events in the order have been completed

 

Order status can be viewed in various locations in EPR, including Chart Review, Patient Care Schedule and the Order History tab. On the Order History tab, double-click on the order to review the Order Status.

Event Flags

Event flags are used to draw attention to abnormal or critical results. Event flags may be seen in the Inbox or Chart Review.

Abbreviations

For common abbreviations used in EPR, please refer to UHN's clinical policy on Use of Abbreviations.

 

Note: Abbreviations, acronyms, symbols, and/or dose designations as indicated on the Institute for Safe Medication Practices (ISMP) Canada's Do Not Use - Dangerous Abbreviation, Symbols and Dose Designations list are not to be used in manual or electronic clinical documentation within UHN.

 

Common Frequency Options and Descriptions

 

When entering an order, the below options can be seen on the Frequency Options screen or the Typical Orders tab. For more information, please see the Frequency Options topic.

 

Note: For medication orders, some but not all of the below options will be displayed. As part of the Medication Clean Up project, some medication procedures have been updated to standards based on ordering best practices, safety and efficiency. As such, Frequency options are customized for each medication. For specific questions on placing medication orders, please consult UHN Pharmacy.

single event Enter a specific date/time
today Today's date
tomorrow Tomorrow's date
unscheduled A single event that has no set date/time
now Current date and time
daily Single occurrence every day
q___d Repeats order every X days, e.g. every 2 days. Can choose odd or even days
q___min Repeats order every X minutes, e.g. every 30 minutes
q___h Repeats order every X hours, e.g. every 4 hours
___ times per day Schedules X number of orders each day
___ times per week Schedules X number of orders for a week following the initial order date. Attempts to spread orders out evenly, no specific times
___ times per month Schedules X number of orders for a month following the initial order date. Attempts to spread orders out evenly, no specific times
qmeal Schedules an order with every meal. Event times are "brk" for breakfast, "lun" for lunch, "sup" for supper
q 8h shift Schedules an order every 8 hours
q 12h shift Schedules an order every 12 hours
for ___ days of week Specify specific days of the week for the order to occur on (e.g. Mondays and Thursdays)
every ___ weeks Repeats order every X weeks
q odd day Schedules orders on odd numbered days
q even day Schedules orders on even numbered days

 

Modifiers

Used in combination with other Frequency options to specific parameters for an order.

as needed As needed (formerly "prn")
scheduled at ___ Used to schedule a specific time for repeating events e.g. "Daily" scheduled at 0900
x ___ Used to specify duration or number of events. Cannot be used for single events. E.g. "qMThs for 3 times"
start at ___ Specify a date/time for a repetitive order to start
stop at ___ Specify a date/time for a repetitive order to stop

 

Conjunctions

Used in combination with other Frequency options to specify parameters for an order

and then Allows you to change the frequency schedule of an order
followed by Allows you to change aspects of an order over time
