Safety Incident?

Any actual or potential event that:

• Is unexpected, unusual or unplanned

• Results in, or could have resulted in, an undesirable

outcome

• Affects, or potentially affects, a patient or visitor

• Is not related to,

– Natural course of the patient’s illness

– Underlying condition

– Routine patient care
