Injuries and Illnesses

What to Report:

 All workplace accidents

 All workplace violence incidents

 All work related diseases and illnesses

When to Report:

 As soon as possible or within 24 hours

To Whom to Report:

 Report to your Manager & complete an Employee

Incident Form

Important:

 If you require treatment from a physician or miss

time from work resulting from a work related injury
/illness, immediately report to Occupational Health

& Safety
