Faxing documents relating to patient care includes, but is not limited to, the following:
· Prescription renewal requests Renewing and Faxing a Prescription
· Public health forms
· CCAC referrals
· UHN infection control
· Documents to a patient
· Documents to insurance company
· Request for most recent bloodwork
· Lab requisition
Note: Any document which is returned to the Records Room
in paper format will be scanned onto a
workstation using either the photocopier or the Kodiak scanners.
1. Select the checkbox to place a green bar beside the desired documents to be faxed.
2. From the File menu, select Fax
3. A
pop up window will appear with options to locate the physician
or allied health providers
that you would like to fax documents to.
4. To select a physician or allied health provider to refer to, there are four options:
Type the provider’s last name (or first few letters of name)
and then click the tab key.
Their name will appear for selection in the Select an Address screen.
Type first few letters of specialty (e.g. cardio)
and then click the tab key.
The specialty will appear for selection in the Select an Address
screen.
Click on the icon located to the immediate left of the One-Time
Address button to open the
entire address book to make your selection.
For providers or locations not listed, enter as a One-Time Address only.
· Be sure to
notify the Quality Improvement Decision Support Specialist (QIDSS) via email to add
the provider to the PSS address book
5. Click OK in bottom right corner of the Address Screen or press enter.
6. The
provider’s name and address that you selected will display.
Click the OK button to proceed.
7. The Select Fax Attachments screen appears. Select the Fax button.
8. The Fax Options screen appears.
· Select the Include Cover Page option
· Use drop down menu to select appropriate cover page, if required
· Ensure the Fax using Internet Fax Account field is selected with appropriate fax
9. Select the Submit Fax Job button.
