Access

Access is required to a number of systems at UHN in order to perform your duties as a clinician. The sections below contain more information on how to obtain access to these systems.

EPR Access

The Electronic Patient Record (EPR) is the primary electronic documentation tool used at UHN and is used to access the eNotes application for electronic Discharge Summary completion.

Dictation Access

At some Physician Services, Discharge Summaries are completed via dictation and transcription.

• Once issued an EPR login and password, Medical Students, Residents, Nurse Practitioners, and Staff Physicians will be able to apply for a dictation number

• This application must be done from a hospital computer (with access to the Intranet)

• Complete the Dictation Access form   in its entirety and submit

• Access is granted within 24 hours (Mon-Fri)


For technical problems, from within the hospital, call:

  • The HELP Desk at ext. 4357 , OR
  • Transcription Services at ext. 6000 (TG, TW and PM), OR
  • Switchboard at ext. 0 and request “Transcription Services” (TR)

  • Nurse Practitioner Students do not require dictation access.

    CliP Access

    Medical Students, Residents, Nurse Practitioners, and Staff Physicians who are EPR users at UHN, are able to access the chart completion application CliP, to view all outstanding Discharge Summaries, and edit dictated Discharge Summaries.

    • Staff Physicians and Nurse Practitioners can also view their chart completion performance via CliP

    • Log into CliP using your EPR credentials or t-ID and password
