A Year in Review 2012 – 2013: Corporate Support
Shared Information Management Services
ConnectingGTA Program
The ConnectingGTA Program is delivering a regional electronic health record for approximately
50 per cent of Ontario’s population. ConnectingGTA, funded by eHealth Ontario and Canada
Health Infoway, will provide new electronic tools for clinicians to access their patient’s health-care
information from other care providers, including hospitals, primary care, home care and
community agencies. This seamless and secure systemwill improve the timeliness of care decisions,
reduce the need for duplicate tests and procedures and improve the patient experience.
The Barcode Patient Identification Band
The Barcode Patient Identification Band helps technologies like the blood glucometers to identify
the right patient to the right products. It will also set the foundation for future UHN initiatives to
help improve patient safety and directly link results to the electronic patient record — improving
accuracy, speed and availability of data to health-care providers.
CLiP
UHN’s Clinical Portal (CLiP) helps Infection Control Practitioners (ICPs) reduce their time to review
patient information by providing electronic information in a consolidated place. CLiP has already
reduced practitioners’ time up to 46 per cent.

Setting a standard
As a result of the project, clinicians can input and access
an extensive amount of patient data from one tool such
as the iPad, instead of having to refer to a number of
separate charts, for example.
Now, clinicians and surgeons have a better look into
patients’ needs and medical histories, which translates
into providing individualized and holistic care to patients
before and after surgery.
In fact, as part of the bariatric patients’ journey, they are
required to follow up with their clinicians over a five-year
period. With this new tool, not only will clinicians be able
to access patients’ medical histories, but they will soon
be able to track and trend each patient’s recovery.
“We’re transforming the way we capture data, and it will
be exciting to see how this will help improve patient
outcomes and enhance research,” said Okrainec. “There’s
no doubt that all the work our team put into this will
become a backbone for other programs at UHN.”
