Below are recommendations regarding the top twelve issues that
the Family Health Team regularly encounters.
Following these recommendations will allow us to care safely for
our patients:
1. When
ordering a prescription, always select the More Details button
to fully display all the fields in the PSS prescription screen.
Multiple medication errors have occurred from skipping this step.
2. Always review the prescription before signing or faxing to ensure accuracy.
3. When faxing scripts to pharmacies, always check that you have selected the correct pharmacy. Some pharmacies have the same four-digit fax number.
4. Many patients have the same name (and address). Are you working in the correct chart?
5. Never leave a chart or a patient list open on your computer monitor when you are away from your computer, for confidentiality reasons. (Not even for a short time)
6. If you are a resident, log in under the correct supervisor.
7. When composing a message in PSS, do not include any items of a personal nature, as all messages are a permanent part of the chart.
8. Enter a progress note after every interaction with a patient (including a phone call).
9. Update the problem list (in the CPP) regularly. This information is automatically pulled into other areas of PSS, such as letters and custom forms.
10. Update your patient’s medication list (in the CPP) regularly.
11. Use PSS messaging to remind yourself of any patient issues that require follow up.
12. Deal with messages and labs in a timely manner. Everyone is expected to review their lab results every 72 hours, according to the FHT policy.
