The Cumulative Patient Profile (CPP)

The Cumulative Patient Profile (CPP) sits at the very top of the Records Menu.

The CPP contains the following sections:

  FH - Family History

  PROB - Current Problem

  HPH - History of Past Health

  Rx - Current Medication/Treatment - lists medications and treatments as they
      are prescribed, changed, renewed, or discontinued

  ALL - Allergies - this field is automatically updated when allergies are added
      manually or when an adverse reaction to a prescription is recorded

  IMM - Immunizations - updated when an immunization has been given

  PERS - Personal Information

  REM - Reminders - not used at the Family Health Centre

  RIS - Risk Factors

·        In PSS, information for referral letters and all automatic forms is pulled from the CPP

·        It is very important to keep the CPP information as up to date as possible. 
We recommend trying to update it at every visit.

·        For clarity, it is important to avoid from using abbreviations.
