TIP SHEET TORONTO REHAB TO ED WORKFLOW
When Toronto Rehab (TR) inpatients have a medical emergency and are sent to the Toronto Western or Toronto General (or Any Other Hospital) Emergency Department (ED), the following process should be followed:
DO
NOT discharge
the TR IP visit in
EPR. TR Inpatient Unit Staff should document an LOA
Checkout procedure, before the
patient is sent to any ED.
Upon TR inpatient arrival at the ED, ED Staff at Toronto Western or Toronto General (or Any Other Hospital) need to create an EP visit for the patient.
*Note an EP visit can be created on a patient in EPR, even if there is an active IP visit.
If the patient does not need to be admitted from the ED to Toronto Western or Toronto General (or Any Other Hospital) for further medical intervention:
ED Staff are to communicate with the TR Inpatient Unit to coordinate the patient's return back to TR.
ED staff should discharge the EP visit.
Upon the patient's return to their respective Toronto Rehab site, TR Unit
Staff should then complete the LOA Check-In procedure
If the patient does need to be admitted to Toronto Western or Toronto General (or Any Other Hospital) for further medical intervention:
ED staff are to connect with the TR Inpatient Unit and communicate the care plan
TR Unit Staff should then discharge the TR IP visit
Staff in the ED are to then convert the EP visit to an acute IP visit
Once the patient is cleared to return to their respective Toronto Rehab site,
the acute IP visit should be discharged by the Acute Unit Staff.
Upon patient return to their respective Toronto Rehab site, TR Unit Staff should create a NEW TR IP visit and admit the patient back onto the Unit.
