Dose Range Orders
As needed dose range narcotics are ordered in a specific format i.e. a minimum dose to a maximum dose within a specified time period. This is for patient safety reasons as it enables EPR to accurately track what is administered and to provide a warning to the practitioner(s) if the maximum dose is exceeded.
Prior to placing new orders, always review the current active orders for the patient to avoid duplication or conflicting orders. These orders can be found by clicking on the Order History tab located within the Order Entry screen.
To order a dose range medication:
1. From within the Order Entry screen, use either the Order Selection or Search tab to find the desired medication. (In this example, the Search tab was used to find the medication.) Once you have done so, click on the medication from the list and then click on the Add Order button to proceed with the order.
2a. You are brought to the Typical Orders tab. Click on the appropriate order based on dose, route and frequency. Then click on the Add Order button.
In the event that the exact order you require is not listed on the Typical Orders screen, you will need to modify an existing order (i.e. the one originally found under Typical Orders) or create an order under the Order Profile tab.
2b. Click on the order that requires modification. Then click on the Order Profile tab to enable you to make the necessary changes to the existing order.
3. Click in the field(s) that requires modification.
4. A list of options is displayed on the screen. To modify the order, you must select the choice that is consistent with the as needed dose range narcotic order. The dose selected must give you the option to enter the minimum and the maximum dose in a specified time frame. Click on the dosage option, and then click on the OK button to continue.
5. You are now prompted to fill in the first blank which is the minimum dose. Type the dose. Click on the OK button.
6. The lower range has been entered and appears as indicated on the screen. You are now prompted to fill in the maximum dose. Type the dose. Click on the OK button.
7. The maximum dose has been entered and now appears on the screen. You are now prompted to enter the cumulative dose time. Type the cumulative dose time (i.e. 1d, 4h, 6h, etc.). In this example, we have entered "4 h" for the cumulative dose time. Click on the OK button.
8. The field has now been modified. The text appears in black to indicate the changed information. Click on the OK button to proceed.
9. The order now appears in the Order Preview area of the Order Entry screen.
10. Click on the Order Summary and the Accept Order buttons to complete the order.
