Patient Dashboard Overview

Located under the Patient Care tab, the Patient Dashboard provides you with a quick snapshot of important patient information.

Alerts will be indicated for Closed Discharged visits, Closed Cancelled visits, Discharged visits and Deceased patients. These alerts will appear directly above the row of icons on the Patient Desktop.

As of April 10th, 2014 new icons were developed based on the International Organization for Standardization (ISO) graphic symbols.

Note: This list does not include ALL icons as new ones are continuously added. In EPR, each icon will have a helpful description beside it (as pictured above), to explain what it means.

	Prohibition/ Restriction	Red Circle with Cross Line: following action should not be taken or must be stopped
	Mandatory Action	Blue Filled Circle: following action should be taken to avoid a hazard; such as an incomplete assessment for current patient visit
	Warning	Yellow Triangle: alert to a hazard that has a moderate patient risk or condition
	Danger	Red Square with White Triangle Inside: severe patient risk or condition
	Supplementary	White Box: additional information to be considered

COVID-19 Vaccination Status		This indicates details of a patient's vaccine status have been entered.
Infection Prevention Control	Contact Droplet Airborne	Flags severe Infection Control Precautions. Contact is the least severe level to Airborne being the most severe. Note: If this information is not relevant for patient, icons will not be displayed.	Header – Danger icon displayed with associated level of precaution (i.e. Contact, Droplet, Airborne). Details: indicate instructions and reason for isolation. I.e. "Private room, gown, gloves, surgical mask and eye protection". “Respiratory illness”.
Allergies/ Adverse Reactions		Reminder Mandatory Action to document Allergies/ Adverse Reaction assessment for patient’s current visit/admission.	Header – Mandatory Action icon with an ‘A’. Details – indicates allergies have not been documented. Previous allergies/adverse reactions will be pre-populated.
		Warning to review patient’s documented allergies/adverse reactions from current admission/visit.	Header – Warning icon with an ‘A’. Details – includes allergens and symptoms. Note: Free-text allergens are identified with an asterisk or yellow highlight on Patient Dashboard. EPR cannot evaluate free-text allergens/adverse reactions for potential interactions; therefore, the appropriate alerts will not appear if ordered for patient. There is no impact on interaction checks or order entry alerts.
		Indicates patient has been assessed at current admission/visit with "No Known" allergies.	Header – Supplementary icon with greyed and crossed out A. Details – allergies are documented as "No Known".
Medication Restrictions		Flags prohibited/restricted medication for this patient.	Header – Prohibition/Restriction icon with image of medication. Details – explanation of medication restriction(s).
Blood Bank Sample Expiry		Indicates there is an overdue "Blood Bank Sample” with either a past due date of more than 24 hours or, with no expiry date at all.	Header – Supplementary icon with an image of a droplet. Note: In absence of a sample, no icon will display.
		Warning to an upcoming expiry of a "Blood Bank Sample” within the next 24 hours.	Header – Warning icon with an image of a droplet. Details – indicate sample’s date/time of expiry.
		Indicates an expired “Blood Bank Sample” (of less than 24 hours ago).	Header – Supplementary icon with greyed and crossed out image of a droplet. Details – reads "Sample EXPIRED".
Alternate Level of Care		Indicates information available around Alternate Level of Care for current admission/visit.	Header – Supplementary icon with an ‘ALC’ image. Details – explained.
Clinical Decision Unit		Indicates emergency department patient as being in a Clinical Decision Unit (CDU).	Header – Supplementary icon with a ‘CDU’ image. Details – explained.
UHN Research Study Patient		Indicates patient is on a Clinical Research Study.	Header – Supplementary icon with an image of a book and apparatus. Details – reads "Patient is on study. For details, click View Clinical Research Studies on the 'Patient Care tab'.”
Preferred Language		Indicates patient’s language preference to be something other than English when receiving care.	Header – Supplementary icon with an image of a person speaking. Note: if preferred language is English, then this icon is absent. Details – indicates preferred language.
Height and Weight		Reminder Mandatory Action if height and weight has not been documented on current admission/visit.	Header – Mandatory Action icon with an image of a weight. Details – indicates height and weight have not been documented for visit. If height is not documented on visit, the message “Unable to calculate, missing height and/or measured weight” appears. Note: similar to allergies, historical weight records will be visible for information.
Height and Weight		Indicates documented height and weight for current admission/visit.	Header – Supplementary icon with an image of a weight. Details – includes height and weight data, along with BSA and BMI information.
Privacy Caution		Indicates documented privacy related cautions for patient.	Header – Supplementary icon with an image of a lock. Details – includes locked information, patient related cautions, and contact person. Information is similar to what is displayed in the "Patient Caution" tab.
Behaviour Safety Alert		Warning to Behaviour Safety Alerts.	Header – Warning icon with ‘B’. Details – includes Behaviour Contributing Factors and information around Management Strategies.
Braden Risk of Pressure Ulcer		Warning that patient has risk on the Braden Scale Risk Assessment.	Header – Warning icon of “Braden Risk of Pressure Ulcer" image. Details – includes the Braden Scale Risk Assessment score (with a total score between 13 and 14) and the statement “moderate risk of developing pressure ulcers".
		Flags patient to be high risk on the Braden Scale Risk Assessment.	Heading – Danger icon with “Braden Risk of Pressure Ulcer" image. Details – includes the Braden Scale Risk Assessment score (with a total score of 9 or less) and the statement "very high risk of developing pressure ulcers".
			Heading – Danger icon with “Braden Risk of Pressure Ulcer" image. Details – includes the Braden Scale Risk Assessment score (with a total score between 10 and 12) and the statement "high risk of developing pressure ulcers".
Advanced Care Planning		Indicates patient has a Power of Attorney (POA) and the copied document has been placed in Chart.	Header – Supplementary icon with "ACP" image and check mark. Details – includes POA Personal Care, POA, Advanced Directive/Living Will and documentation date/time.
Advanced Care Planning		Indicates patient has a Power of Attorney (POA), but did not bring a copy of document.	Heading – Supplementary icon with "ACP" image (and absent check mark). Details – includes POA Personal Care, POA, Advanced Directive/Living Will and documentation date/time.
Morse Fall Scale		Reminder Mandatory Action if the Morse Fall Scale has not yet been documented for current admission/visit.	Header – Mandatory Action icon with an image of a person falling. Details – Reads “Falls Risk not documented on this visit. Please document.”
		Flags patient as High Risk on the Morse Fall Scale.	Header – Danger icon with an image of person falling. Details – includes the Morse Fall Scale score and the statement "High Risk-See standard fall prevention interventions and interventions related to equipment and human resources".
		Warning to patient’s last documented Morse Fall Scale as ‘Moderate Risk’.	Header – Warning icon with an image of a person falling. Details – includes Morse Fall Scale score and the statement "Moderate Risk- See standard fall prevention interventions and interventions related to equipment".
Medication Swallowing Alert		Warning to patient’s requirements to taking oral medication.	Header – Warning icon with an image of a person swallowing medication. Details – includes warning information.
Laryngectomy Alert		Flags patient to have laryngectomy.	Header – Danger icon with a "Laryngectomy Alert" image. Details – includes alert information.
Resuscitation Status		No CPR Full Resuscitation Intubation/No CPR Unknown	There are four code options: No CPR (No resuscitation as per ACLS protocol) Full Resuscitation (As per ACLS protocol) Intubation/No CPR (No resuscitation in the event of cardiac arrest as per ACLS protocol) Unknown (resuscitation status not yet confirmed).
Confusion Assessment Method		Flag for CAM positive.	Indicates that a diagnosis of delirium is suggested.
