Regional Electronic Health Records
The Regional EHR (Electronic Health Records) tab on the Patient Desktop provides access to patient information from external sources. One such source of information, currently in use at UHN, is the ConnectingOntario (cON) application.
The ConnectingOntario program aims to provide clinicians with access to patient data across various hospitals and healthcare centres, to enable streamlined decision-making in patient care. The application houses Acute Care and CCAC data, Lab results, and Diagnostic and Cardiology Reports from multiple contributing sites.
Please note that completion of UHN’s Privacy eLearning Module is a pre-requisite to accessing ConnectingOntario data, and any other Regional Electronic Health Record/s. Also, ensure that you access patient information for only those patients whose care you are directly involved with/whose files you are dealing with on behalf of a clinician.

Shared systems combine personal health information from multiple health care organizations and offer potential access to millions of health records.
Remember that shared systems are to be used strictly to provide care or assist in the provision of care for your patients.
Accessing patient records in shared systems for other purposes, such as research, teaching, program planning, or quality improvement is not permitted.
Failure to comply with these requirements could result in your EPR access being withdrawn indefinitely, as well as additional punitive measures (ie. Dismissal and/or fine). Additional information is available on the ConnectingOntario page under Clinical Tools on the UHN Intranet.
